Book an appointment for advance personal planning

Important information

  • The Public Guardian and Trustee provide an Advance Personal Plan support service.
  • You can book an appointment online to visit the Darwin office for this service.
  • If you need an interpreter, contact the Public Guardian and Trustee by calling 1800 810 979 or email pgt@nt.gov.au.

Make a booking

You'll need to provide your name, email and mobile phone number.

Each appointment covers one Advance Personal Plan. You'll need a separate appointment for each person making a plan.

If no appointments are showing, they have all been booked. You should check back later.

After you book

We'll confirm your appointment via email and SMS. We'll also send you a list of information you need to prepare before your appointment.
