Replace a high risk work licence card

Important information

If your NT high risk work licence card has been lost, stolen or damaged, you can request a replacement online.
You can also apply for a replacement if your name has changed.
You’ll need to provide an ID document. Check the types accepted below.

This takes about 10 minutes.

All steps must be completed in one go. You can't save your application and complete it at a later stage.

You'll need 1 identity document from the list below.

For a change of name you'll also need either a marriage or change of name certificate.

There is a fee. Pay online using a Mastercard or Visa credit or debit card.

Name change requests are free.
