Apply for an NDIS Worker Screening Check

Important information

  • NDIS Worker Screening Checks are for Australian residents who work for registered NDIS providers in risk-assessed roles.
  • If you’re applying as a volunteer, ask the organisation or NDIS participant you volunteer for to complete a Volunteer Declaration Form and upload it with your application.
  • Clearances are valid for 5 years unless revoked.
  • You can apply for a new check or renew an existing clearance using the link below.
  • You'll need proof of identity documents, a selfie photo of you holding ID and your residential address history for the past 5 years. Check the ID and photo requirements below.
  • Get your employer's NDIS employer ID before you start. You can't apply without this.

Before you start

Make sure you understand who needs a worker screening clearance and the rules for applying. For more information, including a complete list of required attachments, go to the NDIS Worker Screening Check website.

You'll need 4 identity documents:

  • 1 category A
  • 1 category B
  • 2 category C.

All documents must be current and valid. The combination of the identity documents must include:

  • your full legal name
  • your date of birth
  • a photograph (provided on one of your identity documents).

If your documents are in different names because your name has been legally changed, you will also need to provide one linkage document as evidence of your legal name change. This can include an Australian marriage certificate or a reissued Australian birth certificate.

Common category A documents

  • Current Australian passport
  • Australian birth certificate
  • Australian visa - supported by a foreign passport
  • Australian citizenship certificate
  • ImmiCard

Common category B documents

Document must show your signature or photo.

  • Category A document you haven't already used
  • Current Australian driver licence
  • Current evidence or proof of age card
  • Shooter or firearms licence
  • Australian marriage certificate

Common category C documents

  • Category A or B documents you haven't already used
  • Medicare card
  • Working with children clearance (ochre card)
  • Financial statement
  • Australian Tax Office Notice of Assessment
  • Australian utility bill showing your name and address
  • Australian private health insurance card
  • Evidence of right to an Australian Government benefit (Centrelink or Veteran’s Affairs)

You'll need to upload a clear 'selfie' photo of you holding your photo ID next to your face.

It must be close and clear so that it's possible to see the details on your ID and compare your face with the picture on the ID.

There is an application fee. Pay using a Mastercard or Visa credit or debit card.
